Payments are due at time of service and you may pay with cash, check, credit card, or an HSA account. Unfortunately, Life Recovery therapists are not contracted with insurance companies at this time. We can provide a superbill which you may present to your insurance company to see if they will reimburse you out of network but there is no guarantee. The therapists at Life Recovery have different fee structures and sliding scales. Adam’s full fee is $120 per session but will adjust his fees based on the total income of your household. You can determine your fee on the chart below.
Identify the row that represents the number of people who are in your family. Then, identify the column that is higher than your combined household income. That will be your fee per session. For example, if you live in a household of 2 people, a combined income of $33,820 or less would pay $30 a session. A combined income of $67,640 for that same 2 people would pay $90 a session.
$30 | $60 | $90 | $120 | |
1 | $24,980 | $37,470 | $49,960
2 | $33,820 | $50,730 | $67,640
3 | $42,660 | $63,990 | $85,320
4 | $51,500 | $77,250 | $103,000
5 | $60,340 | $90,510 | $120,680
6 | $69,180 | $103,770 | $138,360
7 | $78,020 | $117,030 | $156,040
8 | $86,860 | $130,290 | $173,720 |
Cancellation Policy
Life happens and we can’t always make our appointed sessions. We get that. If at all possible, please give 24 hours’ notice when cancelling or rescheduling your appointment. If you can’t give the 24 hours’ notice, we do charge a late cancellation fee. Adam’s late cancellation fee is 40% of your normal rate. If you become ill, or had a crisis happen, or inclement weather, and just can’t make it in, this fee is typically waived. However, if sessions are frequently missed the late cancellation fee will be added to your next session. If more sessions are missed, the late cancellation fee will be required prior to the session starting and normal fees apply after that.
We want to encourage everyone to keep their appointments until they’ve managed to reach their goals. If you do not wish to continue therapy, we would rather discuss ending therapy instead of skipping sessions every week. We also have to run a business and if people frequently cancel, it becomes difficult to schedule everyone.
